Please read each section and item description carefully, and fill out everything to the best of your knowledge.

At the bottom of the page you'll be asked to fill out your full name, and your phone number or email address so that we can contact you to set up a FREE on-site meeting at your business to discuss the project design, requirements, timelines, and options.

There is ABSOLUTELY NO OBLIGATION at all to purchase a website or corporate identity by filling out and submitting this quote request. All information submitted will be handled in compliance with our privacy policy.

Each page or feature below covers most situations, however, being that every website and business identity project is completely custom, every page and feature can be modified or enhanced to support your needs such as: custom programming, rich internet application development, database design and development, and graphic design.

 

1. Website Design

Please select the website design checkbox if you are interested having a new design created for your website, or if you would like your current website redesigned. Also, please select the website hosting checkbox if you currently do not have a hosting account, domain name, or email for your new website. Website Design Description

The Website Design home page will drive the entire design and theme of the website. All pages within the website will match the home page and receive the following:

  • Completely custom designed layout.
  • Full-custom artwork.
  • Unlimited photography with editing.
  • Graphical or text menus with submenus.
  • Unlimited menu items and submenu items.
  • Custom programming for website functionality.
Website Hosting Description

The Website Hosting includes the setup of the following items:

  • Domain name.
  • Hosting account.
  • Email addresses.
  • Secure Socket Layer (SSL).
  • Dedicated IP address.
  • and more.

 

NOTE: All hosting accounts are hosted at GoDaddy.com. All domain names are registered at, or transferred to, GoDaddy.com. The hosting account will be set up in the name of the client or company name, and NOT in the name of Daystrom Creative Group, LLC, its members, or its subsidaries. All payments for hosting services will be submitted directly to GoDaddy via client credit card. All hosting accounts, domain names, email addresses, etc. are the property of the client. Daystrom Creative Group, LLC does not charge hosting fees on top of the GoDaddy.com hosting costs.

Application Development Description

Application development consists of very custom written online programs that perform much more complex functions compared to normal website page functionality. All application development is not covered within the standard list above. You can have as large or small application within your website as you like. If you would like to include application development within your site, please fill out the quote as completely and accurately as you can, and then submit it with the "Application Development" checkbox checked. A representative will contact you within 24 hours to discuss the extra pages in more detail. Examples of applications could be the following:

About Page Description

The About page will contain information describing the company's mission, the history of the company, the company team members, and/or other related company information.

Contact Page Description

The Contact page will contain the phone numbers, fax numbers, email addresses, physical address, driving directions, and/or a map to the office. This page will also contain a form that visitors can fill out and send to your company for more information.

Services Page Description

The Services page will list and describe the services offered by your company. Each item can listed, grouped, or itemized with its title, name, description, image, or price. For services that are to be listed and sold online, a shopping cart will be utilized. Please select the shopping cart checkbox below if you would like to sell items online.

NOTE: $50 for each additional 10 services listed.

Products Page Description

The Products page will list and describe the products offered by your company. Each item can listed, grouped, or itemized with its title, name, description, image, or price. For products that are to be listed and sold online, a shopping cart will be utilized. Please select the shopping cart checkbox below if you would like to sell items online.

FAQs Page Description

The FAQ's page will have up to 10 frequently asked questions that are commonly asked of your company.

NOTE: $50 for each additional 10 FAQs.

Testimonials Page Description

The Testimonials page will list up to 10 testimonials that your company has received.

NOTE: $50 for each additional 10 testimonials.

Terms and Conditions Page Description

The Terms and Conditions page will list and describe the responsibilities, limitations, legal notices, disclaimers, usage, copyright information, and more for your company. Examples of standard Terms and Conditions page can be supplied to your company for reference and/or modification if requested.

Privacy Policy Description

The Privacy Policy page will list and describe the intention of use for visitor and customer information, including visiting, usage, and shopping policies of your company. Examples of standard Privacy Policy can be supplied to your company for reference and/or modification if requested.

Extra Pages Description

Extra pages are website pages that are not covered within the standard list above. You can have as many pages within your website as you like. The average extra page runs $250. If you would like to include any extra pages within your site, please fill out the quote as completely and accurately as you can, and then submit it with the "Extra Pages" checkbox checked. A representative will contact you within 24 hours to discuss the extra pages in more detail. Extra pages may be used for the following and more topics on your website:

2a. Medically Specific Website Pages

Please select the optional physician specific pages that you would like to designed for your website. These pages are exclusive to physicians and dentists, and are designed and tailored to meet the exact needs of your medical practice.

Meet the Doctor Description

The Meet the Doctor page gives an introduction to the doctor(s) within your practice. Information could include years of practice, education, bedside manner, personality, etc. This page is extremely useful in allowing a perspective patient to get to know the you before they enter the office. This generates a level of trust and confidence with you before the initial meeting and helps to establish a comfort level to drive the perspective patient to schedule an appointment.

Meet the Staff Description

The Meet the Staff page gives an introduction to your staff within your practice. Information could include years of experiences, personalities, backgrounds, etc. This page is extremely useful in allowing a perspective patient to get to know the your staff before they enter the office. This generates a level of trust and confidence with your staff before the initial meeting and helps to establish a comfort level to drive the perspective patient to schedule an appointment.

Medical Care Description

The Medical Care page contains information on the type of care provided by your practice. For instance, you may have a OB/GYN practice and would like to provide information regarding both obstetrics and gynecology. You could do so by having a separate medical care page for obstetrics and one for gynecology.

Medical Specialties Description

The Medical Specialties page provides more in-depth and specific information regarding the specialties of your practice. For instance, if your practice is in cardiovascular surgery, you may list specialties in endoscopic surgery, mediastinoscopy, and VATS.

New Patients Description

The New Patients page provides information that new patients would find useful and helpful. This page could provide information regarding copay amounts, scheduling, required forms, and more. This page can supply forms such as individual new patient forms, new patient history forms, insurance policy consent forms, insurance update forms, privacy practice notice forms, and more.

Insurance Plans Description

The Insurance Plans page provides information about your practice as a contracted provider with insurance agencies. The page would provide links to the accepted insurances that would allow patients to check specific plan coverage prior to their appointment.

 

3. Website Add-ons

Shopping Cart Add-on Description

The shopping cart software allows for product purchase online. Each shopping cart that is installed and developed includes, but is not limited to the following features:

  • Multilingual language capability for English and Spanish by default.
  • Support for unlimited products and categories.
  • The ability to contact customers directly via email or newsletters.
  • Printable invoices and packaging lists from the order screen.
  • Orders are stored in a database for fast and efficient retrieval.
  • Customers can view their order history and order statuses.
  • Customers can maintain their accounts.
  • Customers can subscribe to products to receive related emails/newsletters.
  • Accept offline payment processing for checks, money orders, offline credit card processing, etc.
  • Accept online payment processing through PayPal, 2CheckOut, Authorize.net, iPayment, etc.
  • Weight, price, and destination based shipping modules
  • Real-time quotes available through UPS, USPS, FedEx, etc.
  • Flexible tax implementation on a state and country basis.
  • The ability to set different tax rates for different products.

NOTE: A merchant account and payment gateway are required for use with online payment processing through a shopping cart and incur monthly costs. Such costs will be the responsibility of your company. Daystrom Creative Group, LLC does not charge any costs on top of the merchant account and payment gateway costs.

Flash Introduction Add-on Description

Flash introductions are multimedia, animated, introductions to your website. A Flash introduction can be a dynamic aspect to your website and drive people into the website itself. You can promote your products or services through the Flash introduction, or just provide a visually stimulating gateway to your company's website.

Photo Gallery Add-on Description

A Photo Gallery displays either photographs or images of your company's work or products. Photo galleries are especially useful for displaying portfolios for companies that focus on photography, events, landscaping, art, remodeling, etc. All photo galleries are created using Flash technology in order to present the photographs or images at the highest quality and in the most visually appealing aspect.

Forum Add-on Description

A Forum is a web application that's used to hold online discussions and to post user-generated content. Internet forums are also commonly referred to as Web forums, message boards, or discussion boards. The forum would be designed and integrated into your company's website and match the look and feel of the identity of the website and all related print material.

Blog Add-on Description

A blog, or web log, is a maintained area of your website that contains regular entries of commentary, descriptions of events, or other material such as graphics or video related to your company. Entries are commonly displayed in reverse chronological order. Blogs provide commentary or news on a particular subject, or can function as more personal online diaries. A typical blog combines text, images, and links to other blogs, web pages, and other media related to its topic. The ability for readers to leave comments in an interactive format is an important part of many blog. The blog would be designed and integrated into your company's website and match the look and feel of the identity of the website and all related print material.

 

4. Printed Business Identity

Please select any additional business identity items or material that you would like to accompany your website design. It is strongly recommended that a logo and business cards by selected if they are currently not designed. Business Cards Concept and Design Description

Business cards are the first thing that is asked for, handed out, and noticed during a meeting with a perspective client or customer. All business cards are designed from the logo's look and feel to match all print and online material to become part of the corporate identity. All business cards are designed with artwork on both the front and back of the card. This gives the cards a more professional feel and allows for more information on the card, such as appointment reminders, extra contact information, or even better separation of logo artwork and contact information. All business cards are 3.5" x 2" in size and can be set up horizontally or vertically.

Letterhead Concept and Design Description

A letterhead professionally embodies your company's letters, notices, and messages within your company's identity. The letterhead design is derived from the logo's look and feel to match all print and online material to become part of the corporate identity. Letterheads are printed at 8.5" x 11" in full color and on white stock by default. Alternate color stock available upon request.

Envelopes Concept and Design Description

Envelopes act as the branded packaging for your company's letters, notices, and messages that are sent out on company letterhead. The envelope design will be derived from the logo's look and feel, and match all print and online material to become part of the corporate identity. Envelopes are available in either 9" x 12" flat style or 9.5"x4.125" standard #10 style, and will be printed in full color and on white stock by default. Alternate color stock available upon request.

Postcards Concept and Design Description

Postcards contain marketing material, promotional material, and contact information to spark interest or drive prospective clients or customers to your website or business. All postcards are printed in full color on both sides. The postcards will be designed to where they can either be handed out or mailed out through a direct mail campaign. Direct mail services available upon request. All will be designed to be oversized postcards to give greater impact when received in the mail. Oversized postcard sizes are 8.5" x 5.47" and can be set up horizontally or vertically. Standard size postcards are also available by request. Standard sizes are 5.47"x 4.21" and can be set up horizontally or vertically.

Brochures Concept and Design Description

Brochures are used to promote product or service benefits to perspective customers requesting more information in printed a form. Brochures can also be used as a "leave behind" following a sales call or a bid request, or they can be displayed in a holder for clients or customers to retrieve and read at a later time. Most brochures are designed to be tri-fold. They will have 3 panels on the inside and the outside and fold to a finished size of 3.3"x8.5". Other sizes available upon request.

Forms Concept and Design Description

Forms are documents that are used to gather information. Examples of forms are survey's, patient intake forms, questionnaires, and more. The forms can be professionally printed, or created in a PDF format for maximum portability, printability, and downloadability if posted on your company's website. Usage would determine the format.
